Must be between 3 and 30 characters, Must only use letters, numbers, and/or a hyphen., All letters must be lowercase, No spaces
Subscriber Email address:
The email address that should be subscribed to the list
Subscriber Name: (optional)
Your Full Name
Notify subscriber by email:
Sends an email to the subscriber letting them know they were added/removed from the list
Action:	Subscribe yourself Unsubscribe yourself
Subscribe adds the email to the list; Unsubscribe is to remove yourself; Delete is to remove others (requires list manager address)

This form address can be posted with the following parameters:

Manager = Yes/No

Listname = [LISTNAME]

Action = ss/us/so/uo (Subscribe Self / Unubscribe Self / Unsubscribe Others / Unsubscribe Others)

Email = email address

Name = name
